Official Rules - PBR Team Finals Giveaway
1. Sponsor
This giveaway (“Promotion”) is sponsored by OptiWize Health located at 1220 Satterwhite Rd #902 Buda, TX 78610 (“Sponsor”).
2. Eligibility
Open to legal residents of the United States who are 18 years of age or older at the time of entry. Void in any jurisdiction where prohibited or restricted by law. Employees, officers, and directors of Sponsor, its affiliates, subsidiaries, advertising and promotion agencies, and immediate family members (spouse, parents, siblings, and children) and persons living in the same household are not eligible to participate.
3. Promotion Period
The Promotion begins at August 15th, 2025and ends at Sepember 25th at 11:59 Central (“Promotion Period”). Entries submitted before or after the Promotion Period will not be eligible.

Prizes are non-transferable and may not be substituted except by Sponsor in its sole discretion. Sponsor reserves the right to substitute a prize of equal or greater value.
6. Winner Selection
Winner(s) will be selected Randomly from all eligible entries received during the Promotion Period. Odds of winning depend on the number of eligible entries received.
7. Winner Notification
Winner(s) will be notified via email, phone, or social media direct message within 7 days after the Promotion ends. Winner(s) must respond within 5 days to claim their prize. If no response is received within the stated timeframe, Sponsor may select an alternate winner.
8. Taxes
All federal, state, and local taxes on the prize are the sole responsibility of the winner. For prizes valued at $600 or more, winner must complete and return an IRS Form W-9 and will receive an IRS Form 1099-MISC.
9. Publicity Release
By entering, participants grant Sponsor the right to use their name, likeness, city and state of residence, and any statements made about the Promotion for advertising and publicity purposes without additional compensation, unless prohibited by law.
10. General Conditions
Sponsor reserves the right to cancel, suspend, or modify the Promotion if fraud, technical failures, or any other factor beyond Sponsor’s reasonable control impairs the integrity of the Promotion. Sponsor is not responsible for lost, late, incomplete, or misdirected entries.
11. Release of Liability
By participating, entrants agree to release and hold harmless Sponsor, its affiliates, subsidiaries, advertising and promotion agencies, and their respective officers, directors, employees, agents, and representatives from and against any claim or cause of action arising out of participation in the Promotion or acceptance, use, or misuse of the prize, including but not limited to:

Personal injury, illness, accident, or death;

Damage to or loss of property;

Delays, cancellations, or other issues related to booking availability, airline travel, hotel accommodations, ground transportation, or any other travel-related arrangements;

Acts of God, government restrictions, pandemics, strikes, or other circumstances beyond Sponsor’s reasonable control.
Winner assumes all risks and responsibilities associated with accepting and using the prize, including travel and participation in any activities.
12. Platform Disclaimer
This Promotion is in no way sponsored, endorsed, administered by, or associated with Facebook, Instagram, TikTok, etc. You understand you are providing your information to the Sponsor, not to Social Platforms.
13. Governing Law
The Promotion and these Official Rules are governed by and construed in accordance with the laws of the State of Texas, without regard to its conflict of law principles.
